People are dying in prison every day because of "devastating failures" to keep inmates safe, it is being claimed today. New figures show there were more than 600 incidents of self-harm at Devon's three prisons last year. There were also six suicides among the 20 deaths recorded at HMPs Dartmoor, Channings Wood and Exeter. Soaring rates of suicide and self-harm in prisons must be dealt with urgently, the public spending watchdog will say this morning as the revelations are made...
